Procedure
- 01
Confirm the business model and ingredient supply: because ready-to-sell cosmetics may not be freely mixed or subdivided (Article 5(3) of the Act), first confirm the source of base products and raw materials and how quality certificates will be obtained.
- 02
Confirm the preparation manager: a customized cosmetics preparation manager who passed the Article 3-4 exam must be employed, so check the certificate copy and the Article 3-3 disqualifications, and check under Article 8-2(4) of the Enforcement Rule whether the reporter, if a passer, can serve as manager at one shop.
- 03
Prepare the facility statement and floor plan: check that the mixing and subdivision space is separated or partitioned from other uses, and organize the facility statement, floor plan, and photos to match the building register and lease for the shop address.
- 04
Prepare and file the report: attach a copy of the preparation manager's certificate and the facility statement to the customized cosmetics retail business report (Form No. 6-3 of the Enforcement Rule) and submit it by visit, mail, or electronic document to the Regional Office with jurisdiction over the shop.
- 05
Receive the report certificate and review ongoing duties: after the Regional Office confirms the requirements it issues the report certificate; then organize a checklist of the operator's duties (Article 12-2 of the Enforcement Rule) and the change report triggers for operator, name, location, and preparation manager.
